I doubt it was a hang fire, more likely a cook-off from the heat of the barrel.
This accident was caused by a lack of proper training.
The shooter should have followed the MG3 operator manual TDv 1005/019-12, page 83: To prevent a self-ignition in case of a hot-shot barrel when the cartridge does not fire, the bolt must remain locked to the Barrel (5 minutes) before the bolt is opened.
Manuals are cheap, hospitals are not.
